Hydromorphone The name of the drug is called Hydromorphone. The systematic name (IUPAC) is 4,5-epoxy-3-hydroxy-17-methylmor-phinan-6-one. Hydromorphone has a chemical formula of C17H19NO3 . There are four functional groups in Hydromorphone: An Ether group A Ketone group An Amine group (tertiary) A Phenol group Stereochemistry and conformation: Hydromorphone has 4 chiral centres (Four different groups attached to a sp3 hybridised carbon one chiral centre). Therefore it has 24 = 16 stereoisomers. Carbon number 1 has R-configuration Carbon number 2 has R-configuration Carbon number 3 has S-configuration Carbon number 4 has R-configuration Hydromorphone is a semi-rigid molecule because it is composed of a five-member ring system: the phenolic ring (A), the cyclohexane ring (B), the cyclohexanone ring (C), the N-methyl piperidine ring (D), and the tetrahydrofuran ring (E). The molecule has a T shape. Rings A, B and E form a vertical plane where the C and D form the horizontal plane. Ring C exists in a chair conformation due to saturation of C-C double between C7 and 8. Ring D also exists in chair conformation. Synthesis: Commercially, Hydromorphone is made from morphine via either direct rearrangement (reflux alcoholic or acidic aqueous solution of morphine with platinum or palladium catalyst) or reduction of morphine via catalytic hydrogenation, this two reactions both produce Dihydromorphine. The Dihydromorphine then undergo Oppenauer oxidation, where it is oxidised with benzophenone in the presence of potassium tert butoxide or aluminium tert butoxide to form Hydromorphone. Drug stability: The half-life of Hydromorphone is 2.6hours through oral route, where it takes 18.6 hours for sustained release Palladone. Hydromorphone hydrochloride is affected by light, although hydromorphone hydrochloride injection may develop a slight yellowish discoloration, this change does not indicate loss of potency. Hydromorphone hydrochloride injection should be protected from light and stored at a controlled room temperature of 25 degrees C, but can be exposed to temperatures ranging from 15 to30 degrees C; freezing of the injection should be avoided. Hydromorphone hydrochloride tablets should be stored in tight, light-resistant containers, usually at 15 to30 degrees C; however, the manufacturer recommends that the 8-mg tablets be stored at 15 to 25 degrees C. Hydromorphone hydrochloride oral solution should be stored in light-resistant containers at 15 to 25 degrees C, and suppositories of the drug should be stored at 2 to 8 degrees C. Hydromorphone hydrochloride injection reportedly is physically and chemically stable for at least 24 hours in most common IV infusion solutions when protected from light at 25 degrees C. Hydromorphone is slightly soluble in water, freely soluble in alcohol and very soluble in chloroform. Formulation and packaging: In the U.K. Hydromorphone is only available in form of oral capsules and modified release capsules, which means it is released slowly to extend the length of the drugs effect. Therefore, there are 100% 6.31 x 10-5 % = 99.9999% of unionised Hydromorphone which can pass through the membrane barrier and enter the bloodstream. Hydromorphone is highly absorbed by the body in stomach. Mode of action: Hydromorphone is an opioid analgesic; it reacts with the opioid mu-receptors. The mu-receptors are discretely distributed in the human brain with high density in the posterior amygdala, hypothalamus, thalamus, nucleus caudatus, putamen, and certain cortical areas. These receptors are also found in the spinal cord and gastrointestinal tract. It binds onto the mu-receptors and exerts its principle pharmacological effect on the Central Nervous System and gastrointestinal tract to produce analgesia and sedation. Hydromorphone is highly absorbed by the human body and it shows the analgesia effect very quickly once enter the body. Reading Comic Books Essay -- Literary Analysis, Maus, Holocaust
When students learn how to read in elementary school, teachers would teach students how to read comic books and as students we see that the comics would give the animals multiple human traits. Many comic books substitute animals and give them human-like characteristics, such as the ability to talk and walk upright. However, the debate rages on as to what type of animal makes a good character and what type of animal makes a bad character. Comic writers would often use different types of animals that are naturally seen in the real world and they would determine what type of role they would have in the comic book. Also the cartoonist would determine who should prevail, who should not, and their emotions towards one another. In the book called Maus by Art Spiegelman, it tells the story about a mouse named Valdek Spiegelman, who is a Holocaust survivor, and his son Artie. Artie writes a book on his father and the terrifying experience he dealt with when surviving the Holocaust. The book uses animals instead of human characters. The book has three main types of animals that each symbolizes the different cultures during the Holocaust. The Jews are symbolized as mice, the Nazi Germans as cats, and the Poles as pigs. The reason why Spiegelman uses animals instead of humans is because he wants to give the reader a better understanding of what type of animal symbolizes the different types of social classes that are being address. The power hungry Nazis are symbolized as cats because of a catââ¬â¢s constant starvation and tend to be stronger compared to mice and pigs. The Jews are symbolized as mice to show how weak they were and how they were treated during World War II. Spiegelman puts a focus on the mice and the struggle that Vladek went thr... ...he way Ripley talks about comics and animals is how humans have similar traits as animals and act like animals. The traits that come out in some humans are the ââ¬Å"Biology in one personality, the geography location of where one lives, and the facts about one person and how they express it?â⬠This would explain one interpretation of having animals in comics and explains how humans tend to act more like animals. Many different comics have animals in the short stories and interpret the animals in many different ways. Some comics illustrate animals more as simple creatures that are shy and away from humans. Other comics write about animals and how they are treated more like humans with horrible characteristics. It is important to have an understanding of the animal and how it interacts in nature to be able to compare it to a human in a short story comic.

Abortion: Pro Choice Essay
Abortion, the termination of pregnancy before the fetus is capable of independent life, has been practiced since ancient times. With records dating to 1550 BC, itââ¬â¢s no question that abortion techniques have been used throughout the ages as an effective form of birth control. Pregnancies were terminated through a number of methods, including the use of herbs, sharpened instruments, the application of abdominal pressure, and other techniques. In the 19th century, the English Parliament and the American state legislatures prohibited induced abortion to rotect women from surgical procedures that were deemed unsafe. However, in 1973, abortion was legalized as a result of the U. S. Supreme Court rule in Roe v. Wade. This ruling made it possible for women to safely have an abortion, no matter the reason behind it. I wholeheartedly believe that the banning of abortions should not be considered because of the possibility that the womanââ¬â¢s physical health is at risk, the case of rape or incest, or the child would be born with a severe disability. In the case of a pregnancy where the woman is facing physical complications, an abortion should be available if needed. Some examples of physical complications include placenta Previa (the placenta is attached close to or covering the cervix), ectopic pregnancy (the fertilized egg becomes embedded outside the womb), and pre-eclampsia (pregnancy induced hypertension). These life-threatening situations would pose a serious hazard to the mother if the child is carried full term, which means the motherââ¬â¢s life could be spared if an abortion is performed. According to the Guttmacher Institute, fourteen thousand abortions per year are due to rape or incest. If a woman is raped, she should have the right to terminate her pregnancy. In some cases, the baby can not be cared for due to lack of finances, the mother is not mature enough to care for it, or the baby would be born into potentially harmful situations. Oftentimes, the child is not wanted, and would be a potential victim of neglect or abuse. On the other hand, an abortion would only add to the emotional weight placed on the raped woman. Because of this, the mother must make her decision very carefully. If medical tests have proven that the child will be born with severe handicaps that would greatly reduce the quality of life or endanger the fetus, the opportunity to terminate should be available. Allowing abortion because of fetal disability may actually benefit the child in the long run, by saving it from a life full of unnecessary complications. This, however, is a very controversial reason and is not generally accepted by many people due to moral reasons. It is clear that the U. S. government absolutely should not allow the banning of all abortions, regardless of the reason behind it. A choice must be available to prevent the birth of an unwanted child as well as to protect the mother from physical harm. This should essentially be the choice of the mother carrying the child, and if it is in her best interest, her decision to abort should not be influenced by anyone elseââ¬â¢s opinions or moral standards. If the woman has a high risk of physical complications hat would most likely be a result from full term pregnancy, is a victim of rape, or believes that she will be unable to care for a severely disabled child, then the option points that I have made, I urge you to think twice about banning abortion and put yourself in the motherââ¬â¢s shoes. If it was you or your childââ¬â¢s life that was in danger, what would you do? Sit back and let a higher power decide the outcome of your life, or fght for what you believe is the best decision regarding your future?

The History of Sex Discrimination Legislation in the US The United States Constitution did not mention women or limit any of its rights or privileges to males. The word persons was used, which sounds gender neutral. However, common law, inherited from British precedents, informed the interpretation of the law. And many state laws were not gender-neutral. While right after the Constitution was adopted, New Jersey accepted voting rights for women, even those had been lost by a bill in 1807 that rescinded the right of both women and black men to vote in that state. The principle of coverture prevailed at the time the Constitution was written and adopted: a married woman was simply not a person under the law; her legal existence was bound up with that of her husbands. Dower rights, meant to protect a widows income during her lifetime, were already being ignored increasingly, and so women were in the tough position of not having significant rights to own property, while the convention of dower that had protected them under that system was collapsing. Beginning in the 1840s, womens rights advocates began working to establish legal and political equality for women in some of the states. Property rights of women were among the first targets. But these did not affect the federal constitutional rights of women. Not yet. 1868: Fourteenth Amendment to the US Constitution The first major constitutional change to affect womens rights was the Fourteenth Amendment. This amendment was designed to overturn the Dred Scott decision, which found that black people had no rights which the white man was bound to respect, and to clarify other citizenship rights after the American Civil War had ended. The primary effect was to ensure that freed slaves and other African Americans had full citizenship rights. But the amendment also included the word male in connection with voting, and the womens rights movement split over whether to support the amendment because it established racial equality in voting, or oppose it because it was the first explicit federal denial that women had voting rights. 1873: Bradwell v. Illinois Myra Bradwell claimed the right to practice law as part of the 14th Amendments protections. The Supreme Court found that the right to choose ones profession was not a protected right and that womens paramount destiny and mission was the offices of wife and mother. Women could be legally excluded from the practice of law, the Supreme Court found, using a separate spheres argument. 1875: Minor v. Happerset The suffrage movement decided to use the Fourteenth Amendment, even with that mention of male, to justify women voting. A number of women in 1872 attempted to vote in a federal election; Susan B. Anthony was arrested and convicted for doing so. A Missouri woman, Virginia Minor, also challenged the law. The registrars action forbidding her from voting was the basis for yet another case to reach the Supreme Court (her husband had to file the lawsuit, as coverture laws forbid her as a married woman from filing on her own behalf). In their decision inà Minor v. Happerset, the Court found that while women were indeed citizens, voting was not one of the privileges and immunities of citizenship and thus states could deny women the right to vote. 1894: In re Lockwood Belva Lockwood filed a lawsuit to force Virginia to allow her to practice law. She was already a member of the bar in the District of Columbia. But the Supreme Court found that it was acceptable to read the word citizens in the 14th Amendment to include only male citizens. 1903: Muller v. Oregon Thwarted in legal cases claiming womens full equality as citizens, womens rights and labor rights workers filed the Brandeis Brief in the case of Muller v. Oregon. The claim was that womens special status as wives and mothers, especially as mothers, required that they be given special protection as workers. The Supreme Court had been reluctant to allow legislatures to interfere with contract rights of employers by permitting limits on hours or minimum wage requirements; however, in this case, the Supreme Court looked at evidence of working conditions and permit special protections for women in the workplace. Louis Brandeis, himself later appointed to the Supreme Court, was the lawyer for the case promoting protective legislation for women; the Brandeis brief was prepared primarily by his sister-in-law Josephine Goldmark and by reformer Florence Kelley. 1920: Nineteenth Amendment Women were granted the right to vote by the 19th Amendment, passed by Congress in 1919 and ratified by enough states in 1920 to take effect. 1923: Adkins v. Childrens Hospital In 1923, the Supreme Court decided that federal minimum wage legislation applying to women infringed on the liberty of contract and thus on the Fifth Amendment. Muller v. Oregon was not overturned, however. 1923: Equal Rights Amendment Introduced Alice Paul wrote a proposed Equal Rights Amendment to the Constitution to require equal rights for men and women. She named the proposed amendment for suffrage pioneer Lucretia Mott. When she reworded the amendment in the 1940s, it came to be called the Alice Paul amendment. It did not pass the Congress until 1972. 1938: West Coast Hotel Co. v. Parrish This decision by the Supreme Court, overturning Adkins v. Childrens Hospital, upheld Washington States minimum wage legislation, opening the door again for protective labor legislation applying to women or men. 1948: Goesaert v. Cleary In this case, the Supreme Court found valid a state statute prohibiting most women (other than wives or daughters of male tavern keepers) from serving or selling liquor. 1961: Hoyt v. Florida The Supreme Court heard this case challenging a conviction on the basis that the female defendant faced an all-male jury because jury duty was not mandatory for women. The Supreme Court denied that the state statute exempting women from jury duty was discriminatory, finding that women needed protection from the atmosphere of the courtroom and that it was reasonable to assume that women were needed in the home. 1971: Reed v. Reed Inà Reed v. Reed, the U.S. Supreme Court heard a case where state law preferred males to females as administrators of an estate. In this case, unlike many earlier cases, the Court held that the 14th Amendments equal protection clause applied to women equally. 1972: Equal Rights Amendment Passes Congress In 1972, the US Congress passed the Equal Rights Amendment, sending it to the states. The Congress appended a requirement that the amendment be ratified within seven years, later extended to 1982, but only 35 of the requisite states ratified it during that period. Some legal scholars challenge the deadline, and by that assessment, the ERA is still alive to be ratified by three more states. 1973: Frontiero v. Richardson In the case ofà Frontiero v. Richardson, the Supreme Court found that the military could not have different criteria for male spouses of military members in deciding eligibility for benefits, violating the Fifth Amendments Due Process Clause. The court also signaled that it would be using more scrutiny in the future in looking at sex distinctions in the law- not quite strict scrutiny, which did not get majority support among the justices in the case. 1974: Geduldig v. Aiello Geduldig v. Aiello looked at a states disability insurance system which excluded temporary absences from work due to pregnancy disability and found that normal pregnancies did not have to be covered by the system. 1975: Stanton v. Stanton In this case, the Supreme Court threw out distinctions in the age at which girls and boys were entitled to child support. 1976: Planned Parenthood v. Danforth The Supreme Court found that spousal consent laws (in this case, in the third trimester) were unconstitutional because the pregnant womans rights were more compelling than her husbands. The Court did uphold that regulations requiring the womans full and informed consent were constitutional. 1976: Craig. v. Boren Inà Craig v. Boren, the court threw out a law which treated men and women differently in setting a drinking age. The case is also noted for setting out the new standard of judicial review in cases involving sex discrimination, intermediate scrutiny. 1979: Orr v. Orr In Orr v. Orr, the Court held that alimony laws applied equally to women and men and that the means of the partner were to be considered, not merely their sex. 1981: Rostker v. Goldberg In this case, the Court applied equal protection analysis to examine whether male-only registration for the Selective Service violated the due process clause.à By a six to three decision, the Court applied the heightened scrutiny standard ofà Craig v. Borenà to find that military readiness and appropriate use of resources justified the sex-based classifications. Legal Progression of Marriage in America As the British colonies in North America took root, a great part of the economic growth in colonial society was predicated on the labor of imported African slaves. As the number of slaves increased significantly, especially in the southern colonies, a system of separation of the races was established. Since the beginning of the White and Black coexistence in America, Whites have suppressed the rights of Blacks in order to emphasize their racial superiority and to prohibit mixing between the races. In order to maintain their racial purity, Whites established laws making illegal the marriage of Blacks and Whites. Although anti-miscegenation laws were present in early colonial societies, the legacies have continued in the contemporary period. For example, Alabama amended its constitution in 2000 to acknowledge interracial marriages as valid and legal. For over 300 years, anti-miscegenation laws have remained generally the same, outlawing marriages between people of different races. Over time, however, definitions of who is Black have drastically changed, reflecting the status of Blacksââ¬â¢ in society. Anti-miscegenation laws during the era of slavery defined Blacks as having at least one Black grandparent, or one quarter ââ¬Å"Black bloodâ⬠. Later, these same laws during the Great Migration of the twentieth century, evolved to define Blacks by the ââ¬Å"One Drop Rule,â⬠a rule stating that one was Black if he or she had at least one Black ancestor. These laws, as represented in this paper and based on the policies of the colony and state of Virginia, were used to maintain a separation of races and, thus, to preserve the purity of Whites and to guarantee their system of White supremacy. Sent by King James I, The Virginia Company established the first colony in America in 1607 appropriately named Jamestown (History of Jamestown 1). It is generally accepted that the first Blacks were imported to America in 1619, only twelve years after the colony was established. At this point, no specific laws prohibited interracial relations, but societal taboos and religious doctrines were enough to separate Whites and Blacks from sexual contact. Such beliefs were noted in the proceedings between the governor and his council within the colony: In 1630, a man named Hugh Davis, accused of being sexual involved with a Black, was ââ¬Å"to be soundly whipped before an assembly of Negroes and others for abusing himself to the dishonor of God and the shame of Christians by defiling his body and lying with a Negroâ⬠(Hall 602).

The Effects of Obesity in Adults
In today's society overeating can create a vast amount of changes, that may affect lifestyles of daily living. According to CDC, more than one-third of U.S. adults were obese in 2011-2012. The effects of obesity can lead a person down a road, which can be hard to manage in life. A BMI of 30 is about 30 lb overweight and equivalent to 221 lb in a 6'0â⬠³ person and to 186 lb in one 5'6â⬠³ (American Journal). The National Institutes of Health (NIH) now defines normal weight, overweight, and obesity according to BMI rather than the traditional height/weight charts (NIH). People with a body mass index of 30 or more are considered obese, and those with a B.M.I. of 40 or more are severely obese (NYTimes). Obese individuals have a high risk for chronic health conditions and complications. To name a few: hypertension, cardiovascular disease, knee / joint problems, psychological issues, diabetes, and death. Obesity is a measure by body mass index, which is determined by height and weight. If a person gains between 10-20 pounds they are at a high risks of hypertension (OAC). As you gain weight, so does stress throughout the body which causes hypertension. The more food and drink a person consumes in turn will increase their sodium level. Due to the increase of sodium and fat into the diet the endocrine system is forced to compensate and can result in an increase in blood pressure. Hypertension is a chronic condition caused by an increase in blood pressure and is a common complication for obese patients.Abdominal distention is a common complication due to layers of fat that have accumulated on top of muscle. The abdominal wall compressing against the diaphragm causes discomfort for an obese patient and also limits sleeping positions. Sleep apnea is another common complication of obese patients due to the softening of the tissue in the mouth and throat causing the airway to be obstructed. Many of these patients must be placed on a bipap machine while they sleep to help regulate their breathing. Obesity is also associated with knee and joint issues. Increase weigh strains the body, as a person tries to walk with additional weight. Ideal body weight is classified as 50 kilograms for 5 ft. Anything past 5ft is multiplied by 2.3 + 50 to get ideal body weight. For example; a 6'0 ft person ideal body weight would be 50 + 2.3 x 12 equals 77.6 kilograms. Any weight above the average weight is considered obese. Over time the tissues and connective tendons become damaged from the strain of the weight that is bearing down on the knee. Being only 10 pounds overweight increases the force on the knee by 30-60 pounds with each step (John Hopkins). Being overweight increases, the load placed on the joints such as the knee, which increases stress and could possibly hasten the breakdown of cartilage (John Hopkins). Clearly increase weight does have significant impact on walking, causing fatigue, and joint / knee issues.Psychological consequences of being overweight or obese can include lowered self-esteem and anxiety, and more serious disorders such as depression and eating disorders such as binge eating, bulimia and anorexia (MentalHelp). Obese people are made fun of and looked down upon. Obese people have less energy, due to taking more momentum to get their body going. More weight means more force needed to get moving. Hollywood paints an image that everyone around is thin. When it comes to obese people there's a tendency for depression, trying to compare with society. While being depressed obese people have a tendency to want to eat more and lay around doing nothing. Within that mind state leads to eating more and gaining additional weight. Over time, even ordinary tasks of daily life like going up a flight of stairs can lead to exhaustion and a sense of premature aging (Mentalhelp). Diabetes is another consequence associated with being obese. Obese people have a tendency, to eat more sweets, and carbohydrates that turn into glucose. People who are overweight or have obesity have added pressure on their body's ability to use insulin to properly control blood sugar levels, and are therefore more likely to develop diabetes (Obesity Society). Obese people are at high risk of developing type 2 diabetes. Type 1 diabetes, which often occurs in children or adolescents, is caused by the body's inability to make insulin or type 2 diabetes, which occurs as a result of the body's inability to react properly to insulin (insulin resistance). Type 2 diabetes is more prevalent than type 1 diabetes and is therefore seen in roughly 90% of all diabetes cases (Obesity Society). Compared with normal-weight adults, obese adults had at least 20% significantly higher rate of dying with cardiovascular disease (NIH). Plaque builds up due to more fats in diet, which leads to coronary artery disease. As the buildup continues throughout duration of life, so does blockage in the coronary arteries. A block in the coronary arteries with cause decrease cardiac output. With the decrease in blood flow, cardiac tissue death starts to form. Blockage in the heart creates myocardial infarction. Prolong tissue necrosis will lead to cardia death, resulting in death itself. For CVD, death was advanced by 10.3 and 12.8 years in adults aged 45 to 64 years categorized as grades II and III obese (Luisa N. Borrell). Obesity can cause a lot of issues that can be detrimental to activities of daily living. Just as a little of increase weight, the body starts to break itself down. While, normal healthy adult weight is less than BMI of 26, and obesity is anything above BMI 30. A little increase in weight can be life altering. The activities daily living related with obesity decreases in an adult's life. Several different diseases are involved with obesity, most can be cured with just a minor decrease in weight.

INDUSTRIAL ATTACHMENT AT SUNYANI REGIONAL HOSPITAL Essay

I will humbly acknowledge the management and staff of Sunyani Regional Hospital Records Department, especially to Mr. Peter Amponsah Manu, Madam Elizerbeth kyeraah as the Supervisors at Medical Records Department and Mr. Sanyo, the senior manager of Live Records for their guidance, encouragement and also the knowledge impacted which has been a great help. Finally, I would like to thank my parent and family for your support both in prayers and financially throughout this attachment period, for your encouragement and all who helped in diverse ways to make this attachment successful. CHAPTER ONE INTRODUCTION Industrial attachment is one of the requirements needed in order to obtain the Higher National Diploma (HND) Certificate as instituted by the National Accreditation Board. It is a well structured skill training programme forming part of the approved academic standard intended for students to involve themselves, gain practical, managerial and or hand-on experience that pertains the area of course of studies. It also gives the student chance to solve real world problems. For the first two years in school, students have to spend their second semester vacation going through the industrial attachment. Students are required to cumulatively spend a minimum of 24 weeks on the practical industrial training in order to qualify as HND graduates. OBJECTIVE OF THE INDUSTRIAL ATTACHMENT As part of the education reform programme, the polytechnics are upgraded to a tertiary level to provide training and the needed skills to meet the nationââ¬â¢s professional, scientific and technological needs. However, industrial attachment programme has been attached to the academic curriculum where Higher National Diploma (HND) students are allowed to undergo industrial training to enable us to be more enlightened with what is happening in our present day environment to build upon what has been learnt in school. Hence HND students of the Polytechnics are required to serve an attachment with various organizations and write a report to the institution. This attachment in one way or the other creates job opportunities if the participant puts up their best behavior and efforts during their stay in the organization which they could be recommended for a company after their three year courses. It is for this reason that I undertook an industrial attachment at Sunyani Regional Hospital, for two months (from 27th May 2013 to 26th July 2013) at the Medical Records Department. BRIEF HISTORY OF SUNYANI REGIONAL HOSPITAL Sunyani Regional hospital is an ultra- modern hospital which was established in May 2003 and it is located on the Barracks-Techiman road. The hospital was initially opened in 1928 but its bed capacity was around 150. From 1969 to 1972, the then Busia government realized the need for a larger and structurally better hospital the Region and started the construction of the Doctors bungalows, Nursing training college and Nurses quarters to secure the human resource requirement. Dr. Asare said ââ¬Å"after the exist of Busia government in 1972 the project laid of f. In 1983 and 1990 Nanasunyanihene and Dr. C.J Oppong sent several memos and petitions to the succeeding minister of Health on the need for a new Regional Hospital.â⬠Regional capitals like Cape Coast, Ho and Sunyani were announced by the N.D. C government in 1993 to be built to standard and their aim was achieved in 2003 but was named after Prof. John Evans Atta Mills. The hospital is situated on a leveled ground and share boundaries with Penkwase in east and Nkwabeng North in the north. It covers about 450 hectres of land. VISION STATEMENT OF R.H.S To provide a model secondary health care facility for best clinical practice in the country and beyond and also provide the highest quality health care consistent with the service standard. ââ¬Å"We hope to maintain the equipment, estate and other facilities in the hospital to gain the accolade as one of the cleanest, neatest and customer-friendly hospital in Africa.â⬠Said by Dr. Mohammed Bin Ibrahim the Regional Director of Ghana Health Service. MISSION STATEMENT OF R.H.S The main thrust of the health care delivery over years has been to ââ¬Å"improve the overall health status and reduce inequalities in health outcome of the peopleâ⬠. DEPARTMENTS / SECTIONS/UNIT ADMISTRATION. The administration is headed by the administrator who is the next in command after the medical doctor. He administers or checks theà incoming and outgoings of all working and non working staffs of the hospital, new and old suppliers, etc. His ascent on needed requested item or letter permits a transaction between the hospital(procurement unit, account office and suppliers) or activity to take place at the hospital ACCOUNT DEPARTMENT The account unit is responsible for the payments business transactions approved by the doctor and the administrator and the proper accounting of all moneys received or sent out of the hospitals account. The calculation of the total bills of drugs and non drugs to patients after quality health care delivery to enable the hospital claim their money from the government(national health insurance scheme Centre PHARMACY. The pharmacy is where drugs are been distributed to all patient with and without national health insurance card based on the drugs being prescribed by either the doctor or the health assistance in the hospital . LIVE RECORDS. This is where patients folders are stored , retrieved, filed and analyzed in the hospital for easy identification of patient folder , sickness and total turn out of patients at the hospital within a period of time. OUT- PATIENT DEPARTMENT. This is also the place where all patient with and without the insurance cards check their blood pressure before they see the doctor or the health assistance of the hospital. PROCUREMENT/STORES Lastly, this is where the procurement officers/store keeper invites quotation, evaluate and appraise suppliers with the help of the doctor, administrator . Purchases of items are also done by this department and later issues are made to the various departments within the hospital based on their requisitions in their order books according to their needs to the stores department. Ità is also responsible for the control and management of all stock. MAJOR ACTIVITIES The hospital provides the following services to staff and their families as well as other organizations and the organizations the general public: a. Outpatient. b. Inpatient. c. Laboratory. d. Dispensary. e. Maternity and delivery. f. Medicare. g. General surgery. h. A N C/P N C. i. Ultrasound Scan. j. Family planning. k. Public health services. l. Ophthalmic care. m. E N T. n. Ambulance. o. Child welfare clinic. p. Ophthalmic Care. CHAPTER TWO NATURE OF WORK DONE AT MEDICAL RECORDS DEPARTMENT / OUT ââ¬â PATIENT DEPARTMENT (O.P.D) During my industrial attachment at Sunyani Regional Hospital, I was posted to Medical Records Department which constitutes the Out -Patient Department (O.P.D), Filing room and Live Records. At those Departments, the descriptions of nature of work done are categorized under the following: a. Entering patient data in temporal cards. b. Entering information in folders. c. Detecting barcodes of folders and O. P. D numbers. d. Detecting active and inactive insured patient. e. Retrieving of patients folders. f. Registration of new patients. g. Registration of private companies. h. Arranging and sorting of folders on the shelves. i. Tallying of Out-Patient Department attendance. j. Entering daily ward states in Microsoft excel. k. Entering the monthly statistical diagnosis in Microsoft excel. l. Filling of claim forms. DETAILED ACCOUNT OF THE TRAINING/WORK PERFORMED BY THE STUDENT. ENTERING PATIENT DATA IN TEMPORAL CARDS [T.C]: This are cards issued to patients as their first visit in hospital to take medical treatment, with their names, ages, date of birth, contact address, insurance numbers etc and also they are giving Identification cards with numbers to have access to their temporal cards. ENTERING PATIENT INFORMATION FOLDERS: Patients whoââ¬â¢s Temporal Cards are full with history, are assigned by Doctors to retrieve folders from the Out- Patient Department (O.P.D) before treatment which was my duty. DETECTING BARCODES OF FOLDERS AND O.P.D NUMBERS: Internet application software likes e-Archive and Hams are used at the Out-Patient Department (O.P.D) in detecting the barcodes of folders when patient issue their Identification Cards with O. P. D numbers. When patient displace their cards, I use their insurance numbers or sur names to search for their O. P. D numbers for them to reach their fol ders before medication. DETECTING ACTIVE AND INACTIVE INSURED PATIENT: All patients insured are to issue their National Health Insurance I.D cards for verification of either active or inactive by using Oracle internet application software at the O. P. D before they consult the Doctors. RETRIEVING OF PATIENT FOLDERS: This involves picking of folders of patient from the folder shelves using their barcodes and Out Patient Department (O.P.D) numbers on their hospital cards. After picking the folder, you enter the name of patient and the O.P.D number in the Admissions and Discharges register before you give the folder to the patient and later proceed to make entries into the computer at the filing room. REGISTRATION OF NEW PATIENTS: Patients who has just come to the hospital for the first time are registeredà by the use of HAMS. Their details like their names, gender, age and Home address are recorded in the new case records book. Also, through interviews, relevant information like date of birth, marriage status, religion, contact number, Postal address, including others are recorded in the folder and given an O. P. D number folder number. A hospital card bearing the patient name, O.P.D number, date of registration is made, added to the new folder and handed over to p atient only when he or she comes to the hospital. A SAMPLE OF PATIENT OF FOLDER REGISTRATION OF PRIVATE COMPANIES. Currently, Sunyani Regional Hospital has approved services to private Insurance companies such as Premier Health Care, MEDI- X Health System, Nationwide Mutual Health Care, Newmont Gold Ghana Limited, and among other private insurance company. Registration of patients with private insurance card is done by recording the name of the patients, date of birth, insurance number ,age, gender, name of scheme , claim number, Hospital Record number of patient are recorded in private insurance form or claim form book before a company patients can received a Medical Services. ARRANGING AND SORTING OF FOLDERS: Sorting of folders implies arranging folders in order by using their barcode numbers and O. P. D numbers on the shelves in the filing room after the patients has been treated. TALLYING OF OUT- PATIENT DEPARTMENT ATTENDANCE: This process was done at the Live Records to assist the hospital know the number of patients attendance. The Tally Sheet is categorized into male and female and ages, t hus 0-4years, 4-10years, 10-18years, 18-30years,â⬠¦ 60years and above including the total on sheet before entries are made Microsoft Excel. ENTERING DAILY WARD STATES IN MICROSOFT EXCEL :The Daily Ward State like number of Admissions, Discharges, Bed occupancy in wards such as Accident and Emergency (A and E),Male/Female medical, Male/Female surgical, Male/Female ward etc were entered in Microsoft Excel spreadsheet at Bio-statistics/Live records. ENTERING THE MONTHLY STATISTICAL DIAGNOSIS IN MICROSOFT EXCEL: After the summary of Diagnoses recorded at each ward or clinic in the monthly morbidity book, the data is entered in Microsoft excel spreadsheet, the sheet is grouped intoà gender, age and in diseases like Eye cataracts, Reflective eye, U T I, Malaria, Anemia in pregnancy, polio, scrotal diseases, cardio respiratory diseases etc, after the is data cross examined it is link to Ghana Statistical web. CHAPTER THREE A. NEW KNOWLEDGE/SKILLS ACQUIRED, AND CHALLENGES ENCOUNTERED NEW KNOWLEDGE/SKILLS ACQUIRED AT MEDICAL RECORDS DEPARTMENT. My presence at the Medical Record Department, I acquired so many skills and new knowledge of which are helpful in my studies. Among these are: (a) I develop a good human relationship with staffs and patients. (b) The programme made my typing speed to increase when entering data. (c) It enhances my technology when workings with Microsoft excel spreadsheet especially dealing with calculations. (d) I had ability to work overtime and withstand any pressure with presence or absence of staffs. (e) I acquire knowledge and experience in working different internet application soft wares. (f) More skills were gain in filling of different claim forms and how to register prisoners before medication. CHALLENGES ENCOUNTERED AT MEDICAL RECORDS DEPARTMENT. Sometimes, three or four staff from various sections may come to the Health record room at a go all requesting for their folders. Since I was not perfect with the location of almost all the folders in the room, I had to struggle in other to serve them. I had to adjust to sitting and working at the same place for eight hours which was quite difficult and stressful because I had to follow strictly to the procedure at work by reporting very early and closing in the evening. My shirts get dirty anytime I work in the filing room. The folders are always dusty so when retrieving and filing them on the shelves makes my shirts become dusty. I had to wake up at dawn in other to join the companyââ¬â¢s bus; if I miss the bus then I have to use my own money as transportation. Feeding was a bigà blow on me, because the staffs were fed whilst the attachment trainees were not. B. CONTRIBUTION OF STUDENT TO ORGANIZATION During my industrial training Sunyani Regional Hospital, I participated in almost every activity that was undertaken at the medical record department which included; Issuing barcodes numbers on newly patient folders. Filing of folders into their respective position on shelves at the filing room. Filling of claim forms at the O. P. D. Entering of daily ward states in Microsoft excel.à Books, folders and other necessities needed at out-patient department to record data were pulled on wheel from the stores. C. GENERAL COMMENTS, OBSERVATION AND IMPRESSION GENERAL COMMENTS. Upon all the challenges I adopted new skills in working overtime with both senior and junior staffs. I had ability to work without anybodies supervision at the field of work. More experience and technology was attained in any section or department I was posted to. OBSERVATIONS. I observed the following during my period of attachment that: The management of the hospital for instance the Medical Record Department is keeping good records. Computers available at their disposal, those not meet the so called ââ¬Å"standard computerized hospital.â⬠The compound of the Hospital is enticing especially the lawns, building structure and location of the Nursing Training College. Quality food is given to the patients on sick bed with good health care delivery. Marketing of shoes, perfumes, ladies tops etc is done by some staffs, and feeding is inadequate to meet all staffs. Mixing of patients folders. After treatment all folders are kept in the filing room but patients may be after their folders for treatment but can not find them. IMPRESSION. I was so impressed of how the supervisors welcome me and trained me to be equipped on the field of work. Also, I wonder how the staffs socialized themselves with me; patients may think I am a permanent staff. CHAPTER FOUR CONCLUSION School authorities should create more avenues for attachment trainees because it aid students to be well equipped in the field of work. It also serves as a field where students put into practice of the theoretical aspect leant from school. It should be encourage as far as Higher National Diploma is concerned. RECOMMENDATION. I recommend that staffs who market at the hospital should be prohibited. Attachment trainees should be motivated. I also recommend that proper measures should be taking in keeping patients folders. Lastly, food should be equally distributed to all staffs at the Hospital.
